HealthPathways Sydney
- Moved from the longstanding generic password and login process to individual accounts and more secure access. This enables users to set their login credentials for more than one HealthPathways region and enables more accurate recognition of user engagement with the platform and benchmarking with other HealthPathways regions.
- Over 1350 individual accounts were registered this year.
- Introduced user activity recording enabling self-reflection and CPD reporting.
- Remains the lead Australian HealthPathways site in terms of locally relevant clinical and referral information, with 1046 published pathways.
- Completed the NSW Health Agency for Clinical redesign program that has enabled the introduction of a streamlined content renewal processes to enhance clinical content safety checking.
- Continued the deployment of HealthLink GP and allied health professional eReferral to SLHD clinics and services.
- Commenced the LHD wide adoption of the Engage Outpatients platform for Referral management, in collaboration with SLHD Digital Health and Innovation.
- Active participation and implementation of NSW Health Statewide Referral Criteria (SRCs).
- Introduced new content to support the introduction of the National Lung Cancer Screening Program.
- Introduced the National HealthPathway for Metabolic Associated Fatty Liver Disease (MAFLD) in collaboration with the University of Sydney and the Gastroenterology Association of Australia (GESA).
- Supported Model of Care changes across SLHD services.

HealthPathways South East Sydney
- 655 live pathways.
- 65 pathways reviewed and 40 pathways localised since 1 July 2024.
- Transitioned to individual log ins, providing users with access to CPD reports and an improved search experience.
- Promoted relevant pathways at CESPHN CPD events.
- Continuing to align pathways with State-wide Referral Criteria and eReferrals as released.
- Ongoing development of lead region pathways for HIV and menopause.
- Undertaking research on local GP use of HealthPathways.
- Supported model of care changes across SESLHD services.
Usage of HealthPathways South East Sydney
Primary Health Networks – Aged Care and Dementia Clinical Referral Pathways
Over the last two years Central and Eastern Sydney PHN has reviewed, developed, and enhanced its aged care and dementia clinical and referral pathways relevant to the health needs of the central and eastern Sydney region and in response to the Aged Care Royal Commission. These pathways are already being used by clinicians during consultation with patients to support assessment and referral to local services and supports.
Where existing aged care HealthPathways were already in place, these pathways have been reviewed and updated to improve the pathways and ensure currency, accuracy, and consistency with current medical best practice, as well as providing linkages to local services.
In the last 12 months, the Sydney and South Eastern Sydney HealthPathways teams have continued to review, develop, and enhance aged care and dementia pathways across four main areas:
- Cognitive impairment and dementia
- Maintaining healthy ageing
- Residential Aged Care.
- Aged Care Pharmacology
The project covered over 45 pathways, including the development of 15 new pathways.
