Integrated Team Care

The Integrated Team Care (ITC) Program aims to contribute to ensuring the Aboriginal and/or Torres Strait Islander peoples enjoy long and healthy lives in accordance with target 1 of the socio-economic targets of the National Agreement on Closing The Gap. 

First Nations Australians have higher rates of chronic disease and are 2.5 times more likely to have an avoidable hospital admission. 

Chronic conditions require complex care across various types of health services. This program helps patients understand and manage their health. Support is individualised to help people to access the health care they need. It also helps health services provide culturally appropriate care. 

Achievements

Sydney Local Health District, South Eastern Sydney Local Health District and Sydney Children’s Hospital Network Aboriginal chronic care teams have continued to provide this service. 

ITC provides supports First Nations clients with chronic diseases by:   

  • Clinical care coordinators and/or Aboriginal Outreach workers aiding clients to navigate their needed supports and services.
  • Providing assistance with organsing and/or funding transport to medical appointments.
  • Assisting with funding support for supplementary services for chronic disease needs including specialist and allied health appointments and specific medical aids/appliances.
  • Supporting clients with a better understanding of their condition and self-management strategies.
